How health information may be used
MedSuao may use and disclose protected health information for treatment, payment, and healthcare operations. Examples include coordinating care with another treating professional, submitting claims, reviewing service quality, conducting compliance activities, and operating the practice.
Information may also be used or disclosed when permitted or required by law, including for public-health and safety activities, health oversight, workers' compensation, organ donation, certain law-enforcement purposes, judicial or administrative proceedings, and to prevent or reduce a serious and imminent threat. Only the minimum necessary information will be used or disclosed when that standard applies.
Uses that require written authorization
Uses and disclosures not otherwise permitted or required by law will be made only with your written authorization. You may revoke an authorization in writing at any time, except to the extent MedSuao has already acted in reliance on it.
MedSuao will not sell your protected health information or use it for marketing in a way that requires authorization without first obtaining your written permission.
Your rights
Subject to applicable law, you may exercise the following rights regarding your protected health information. Some requests must be submitted in writing, and reasonable cost-based fees may apply where permitted by law.
Inspect or obtain an electronic or paper copy of your health information
Ask MedSuao to correct information you believe is incorrect or incomplete
Request confidential communications at a particular address or by a particular method
Ask MedSuao to limit certain uses or disclosures
Request an accounting of certain disclosures
Choose someone to act for you when legally authorized
Receive a paper copy of this notice at any time
MedSuao's responsibilities
MedSuao is required by law to maintain the privacy and security of protected health information, provide this notice of its legal duties and privacy practices, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required by law.
MedSuao may change this notice and make the revised notice effective for information already maintained as well as information received in the future. The current notice will be posted on this website and will show its effective date.
